Yesterday I was able to work in the premie room finally. The little baby (her name is Claire) that I put the feeding tube in was at the clinic receiving a blood transfusion. She was supposed to get 75ml total but after 15ml had infused her IV went bad. The nurses at the clinic had tried several times to restart it but were unsuccessful. So the manager of the orphanage asked if I would be willing to go try. When I arrived at the clinic I thought the nurses there knew I was coming to start the IV, but they told me that they didn't want to try anymore that day because she was too dehydrated and her veins were bad. We were about to bring her home to the orphanage and one of the nannies remembered she still needed her antibiotic which is usually given IV. So the nurse said she would give it IM (as a shot in her leg). When she drew up the medication she had 1.5ml of the medicine in a syringe that she was about to give. I told her that was too much volume to give a baby IM. She should only give 0.5ml in one site. So she reconcentrated it to give 0.5ml. As she was giving the shot she asked how long I'd been working with Watoto and if I was from the states. I told her I've been here one week and I'm a neonatal nurse. Then she was surprised. She didn't realize I was a neonatal nurse. She thought the nanny was expecting her to restart the IV again after she had tried too many times. She said she's used to adults and big kids and if she had known I was a neonatal nurse she would've let me put the IV in sooner because she felt bad about giving the shot. Then she let me start the IV which I was able to start on the first try. So the baby was able to finish her blood transfusion that night.
Claire is back home today and still had the IV in to receive her antibiotic. The manager asked me if I would be willing to give her medicine at the orphanage so they don't have to bring her back and forth to the clinic. When I was going to give the antibiotic today her IV wasn't working and I had to restart it again. Fortunately, I was able to restart it on the first try.
